Chapter 6 of the Teen Transition Planning Guide contains several “Key Domains”; these are different areas of a teen’s and family’s life that will likely need to be explored at some point in a family’s journey.

Phoenix Human Services provides a Self Help Skills (SHS) 9 month or Self Help Extended Services (SHSE) 18 months, programs designed for either individual or group settings for those individuals living with family or caregivers.
